Chronic disease is emerging earlier as employers face growing health and benefits pressures, underscoring the need for prevention, early intervention and connected care
TORONTO, Oct. 29, 2026 /CNW/ -- A new GreenShield report provides evidence that connecting coverage and care can help identify chronic disease earlier, improve health outcomes and deliver measurable value for employers and members. Among members living with chronic disease who accessed GreenShield health services, the company's integrated payer-provider model generated $1.43 in value for every $1 invested through reduced mental health and chronic disease claims alone. Potential additional value from reduced absenteeism or disability would be incremental to the $1.43.
Drawing on insights from nearly eight million members, the report, Rethinking Chronic Disease: The Growing Role of Preventive and Integrated Care, examines the growing prevalence and complexity of chronic disease and the impact of earlier, more connected care.
The report highlights a growing challenge for employers and health systems alike: chronic disease is no longer primarily a later-life issue. More members are developing chronic conditions earlier in adulthood, while an increasing number are managing multiple health concerns simultaneously.
Nearly one in four members in early adulthood is living with at least one chronic disease, while members in middle adulthood are four times more likely to be managing multiple chronic conditions than those in younger adulthood.
These trends suggest that traditional treatment-focused approaches may no longer be enough. Instead, employers may need to invest earlier in prevention, personalized support and proactive care to help employees maintain their health before conditions become more complex.
"Chronic disease is no longer a challenge that begins later in life, and our health system continues to be structured around responding once people become sick," says Yan Wang, Senior Vice President, Product, Experience and Health Outcomes at GreenShield. "By connecting coverage and care, we can identify needs earlier, connect members to personalized support and measure what is actually improving their health."
Key insights from the report:
- Chronic disease is emerging earlier and becoming more complex:
- Nearly one in four members in early adulthood is living with at least one chronic disease.
- Middle-aged adults are four times more likely than younger adults to be managing multiple chronic conditions.
- Earlier, personalized intervention improves outcomes:
- Nurse-led diabetes support delivered more than twice the rate of improvement in blood sugar control.
- Participants in GreenShield's Personalized Nutrition program improved their diet quality, as measured by scores associated with risk factors for cancer, heart disease and obesity.
- Integrated care creates measurable value:
- Forty-three per cent of members living with chronic disease also accessed mental health medications or services.
- Members receiving specialized counselling reported improved well-being after three sessions.
- Members who accessed GreenShield health services through the integrated payer-provider model generated $1.43 in value for every $1 invested.
- Value was driven by reduced mental health and chronic disease claims.
"For employers, the opportunity is to move from reacting to illness to actively supporting health," adds Wang. "Using health and benefits insights to intervene earlier and connect employees with the right care can improve outcomes while helping employers get more from their benefits investment."
Advancing a More Integrated Approach to Chronic Disease
As chronic disease becomes a larger driver of health care and benefits spending, employers face an important question: how can they better support employees before health challenges become more complex and costly?
Traditional health benefits models often separate coverage from care, making it difficult to identify emerging needs, coordinate support and measure outcomes across the full health journey.
Through GreenShield+™, Canada's first integrated health care and insurance ecosystem, GreenShield brings together benefits coverage with services such as nurse-led chronic disease management, personalized nutrition counselling, mental health support and care navigation to help address this challenge.
By connecting benefits data, health insights and care delivery, GreenShield can identify opportunities for earlier intervention, guide members to personalized support and gain a clearer understanding of which interventions are driving better outcomes.
